That explains the dangerous, but not the disgusting. I don't have the same "eww" reaction to eating in bed, even though I don't really like crumbs in my sheets.
Surely it'd make the sheets/bedclothes smell horrible! Smoking in bed is one thing, sleeping/waking with the smell of stale smoke and the sight of yellowy sheets with cigarette-burns in them just seems positively minging. Though I suppose if you smoke then your clothes/hair always pong & you probably have very little sense of smell left anyway.
(No, I don't smoke. But I wasn't sure which option best described "tried it as a teenager, natch; guiltily enjoyed the habit/image, but couldn't really get into the mouthful-of-ick and clothes-always-stinking aspect of it".)
Then again, I think smoking in the street is incredibly uncouth, and can't understand why the Government is forcing people to do it.

The second question is one of those polls where I want to tick more than one options - I do think smoking's disgusting anywhere, but I also think it's worse in bed.
I still love the irony of the fact that I am the only non-smoker in my entire immediate family, yet I am the one who suffers the collapsed lung and ends up fighting for my life in hospital...

It's definitely past tense, so 'occasional' doesn't seem to fit - and both "ex-smoker" and "never have" seem a bit misleading.
